To improve service quality, it is necessary to survey the level of patient satisfaction. Puskesmas Penagan and Puskesmas Batu Rusa are accredited health centers in Bangka Regency, Bangka Belitung Province. Puskesmas Penagan holds a”Madya” accreditation, while Puskesmas Batu Rusa holds a “Paripurna” accreditation. This study aims to assess patient satisfaction with pharmaceutical services at Puskesmas Penagan and Puskesmas Batu Rusa in 2021 based on five dimensions of satisfaction: reliability, responsiveness, assurance, empathy, and tangibles. This research design was cross sectional with the servqual method and the Mann-Whitney statistical test for the difference of two independent samples. Respondents consisted of 366 individual, with 171 patients from Puskesmas Penagan and 195 patients from Puskesmas Batu Rusa. The results indicate that patient satisfaction at Puskesmas Penagan has an average score of 3.85 for reliability, 3.74 for responsiveness, 3.68 for assurance, 3.74 for empathy, and 3.88 for tangibles. In comparison, patient satisfaction at Puskesmas Batu Rusa scored 3.86 for reliability, 3.78 for responsiveness, 3.85 for assurance, 3.91 for empathy, and 3.85 for tangibles, out of a maximum score of 5. The Mann-Whitney test revealed a significant difference in patient satisfaction with pharmaceutical services, with a p-value less than 0.05. Puskesmas Batu Rusa showed higher satisfaction levels in the indicators of staff’s problem-solving ability, speed of service, and knowledge in providing drug information. Conversely, Puskesmas Penagan demonstrated higher satisfaction in the indicators of drug safety, staff attentiveness to medication needs, and the layout of the pharmacy area.</p> Nurita Andayani, Sondang Khairani, Rasta Naya Pratita, Robi Ahdi Copyright (c) 2024 Indonesian Journal of Administrative and Clinical Pharmacy https://journal.univpancasila.ac.id/index.php/IJACP/article/view/6718 Fri, 28 Jun 2024 00:00:00 +0000 Clinical Evaluation of The Radiopharmaceutical 99mTc-Ethambutol to Diagnose Pulmonary and Extra-pulmonary Tuberculosis – Case Series https://journal.univpancasila.ac.id/index.php/IJACP/article/view/6644 <p>Infectious tuberculosis disease is caused by Mycobacterium tuberculosis and can infect the lungs as well as other organs (extrapulmonary). Advanced diagnosis of TB disease using nuclear medicine methods involves the use of the radiopharmaceutical <sup>99m</sup>Tc-ethambutol. This study aims to clinically evaluate <sup>99m</sup>Tc-ethambutol for diagnosing both pulmonary and extrapulmonary TB. Research methodology is clinical scientific research based on the disciplines of nuclear medicine and nuclear pharmacy in marketing surveillance of new drugs. The research period was in 2023, conducted at multiple centers across three hospitals with a sample of 22 serial post-marketing surveillance cases. The examination results for TB in the subjects using the ethambutol patch test showed positive results in 40.9% and negative results in 59.1%. Comparative tests of the ethambutol patch test against treatment outcomes using X-ray methods yielded positive results in 40.9% and negative results in 59.1%, and using culture methods yielded positive results in 27.3% and negative results in 72.7%. The comparative results of TB examinations using the three methods showed the same results and were compared with treatment outcomes, with 6 subjects testing positive for TB, 13 subjects testing negative for TB, and 3 subjects with positive results being cases of extrapulmonary TB. All subjects diagnosed with TB underwent treatment. The measurements of the ethambutol patch test method using scintigraphy showed a sensitivity of 100%, specificity of 100%, and accuracy of 100%. The study concluded that the clinical evaluation of <sup>99m</sup>Tc-ethambutol can be used to diagnose both pulmonary and extrapulmonary TB. No drug side effects or subject complaints were found before or after the injection of <sup>99m</sup>Tc-ethambutol. The formulary consists of 36 types of drugs and is only applicable within the regional institution. The pharmacoeconomic study was conducted using a cost-effectiveness analysis method for the proposed formulary drug list in the analgesic therapy class. The therapeutic effectiveness of each drug is expressed as a percentage, calculated by dividing the number of cases for each drug by the total number of cases in 2018. The samples analyzed were ibuprofen suppositories, paracetamol suppositories, ketoprofen suppositories, ketorolac injections, and tramadol injections. The results of the pharmacoeconomic studies on analgesic drugs indicated that ibuprofen suppositories are highly effective and cost-effective. Despite this, the Tangerang City Health Department does not currently include ibuprofen suppositories in its formulary. Instead, they have chosen ketorolac for injection. However, based on the ICER calculation, it has been determined that the procurement cost of ketorolac injection exceeds RP99.19 per case compared to tramadol injection. Only ketoprofen suppositories are deemed suitable and eligible for inclusion in the Formulary. The lack of trained personnel in pharmacoeconomic studies meant that this crucial study was not considered important. This study aims to evaluate the prescription services and determine the level of satisfaction of outpatients at the pharmacy facility of Pesawaran Regional General Hospital. This study uses a descriptive cross-sectional method by randomly observing 200 prescriptions that enter the pharmacy. Evaluation of prescription services with service indicators and satisfaction levels by distributing questionnaires filled by outpatients using variables of reliability, responsiveness, trust, empathy and direct evidence. The results of the observations were: checking the patient's name and identity 100%, checking the prescription 91.5%, counselling 0%, delivering the drug according to the prescription 85.5%, changing the type of drug 7%, completeness of the etiquette 90.5%, drug preparation time for compounded drugs was 18.29 minutes and for non-reconstituted drugs 13.54 seconds, final check 94.5%, documentation 95%, service according to protocol 100%. The results of the questionnaire on the level of satisfaction of outpatients with prescription services showed that 68 respondents (34%) were satisfied and 132 respondents (66%) were very satisfied. It can be concluded that prescription services have not met the pharmaceutical service standards according to Ministry of Health Indonesia Regulation (Permenkes) No.72 of 2016 with a satisfaction level that is, the patient is very satisfied with the prescription services provided at the pharmacy department of Pesawaran regional general hospital, Lampung Province. </span></p> Nurita Andayani, Sondang Khairani, Sari Royani Copyright (c) 2024 Indonesian Journal of Administrative and Clinical Pharmacy https://journal.univpancasila.ac.id/index.php/IJACP/article/view/6719 Fri, 28 Jun 2024 00:00:00 +0000 Risk Factors for the Incidence of Overdose of Etoricoxib Drug in Outpatient at Hospital in Indonesia https://journal.univpancasila.ac.id/index.php/IJACP/article/view/6673 <p>Etoricoxib is a non-steroidal anti-inflammatory drug (NSAID) commonly prescribed for the treatment of various inflammatory conditions. However, the risk of overdose associated with the use of etoricoxib in outpatient polyclinic settings remains a significant concern. This study aimed to investigate the potential risk factors contributing to the incidence of etoricoxib overdose in outpatient polyclinic patients. A retrospective observational study was conducted, analyzing data from medical records of patients who had experienced etoricoxib overdose in outpatient polyclinics. The study population included patients who had been prescribed etoricoxib and presented with signs of overdose. The analysis revealed several potential risk factors contributing to the incidence of etoricoxib overdose in outpatient polyclinic patients. Factors such as age, multiphatology and polypharmacy were found to be associated with an increased risk of overdose (P value = 0,001).
